FLORIDA SPRINGS 2027 — Trip Contract

I acknowledge that participation in scuba diving and travel activities involves inherent risks. I voluntarily release and hold harmless Triangle Dive Shop/Water World, its owners, guides, agents, staff, contractors, and affiliated providers (Collectively “Released Parties”) from any and all liability for personal injury, property damage, loss, accident, or death arising from participation in this trip, whether due to negligence or otherwise.

I understand and agree that neither Triangle Dive Shop/Water World Inc, TDI/SDI, nor PADI Americas, Inc., nor its affiliate or subsidiary corporations, nor any of their respective employees, officers, agents, contractors or assigns (hereinafter referred to as “Released Parties,”) may be held liable or responsible in any way for any occurrence on this trip which may result in personal injury, property damage or wrongful death or other damages to me, my family, estate, heirs or assigns that may occur as a result of my participation in this trip or as a result of the negligence of any party, including the Released Parties, whether passive or active.

I understand the terms herein are contractual and not a mere recital, and that I have signed this Agreement of my own free act and with the knowledge that I hereby agree to waive my legal rights. I further agree that if any provision of this agreement is found to be unenforceable or invalid, that provision shall be severed from this agreement. The remainder of this agreement will then be construed as though the unenforceable provision had never been contained herein.

I understand and agree that I am not only giving up my right to sue the Released Parties but also any rights my heirs, assigns, or beneficiaries may have to sue the Released Parties resulting from my death. I further represent I have the authority to do so and that my heirs, assigns, and beneficiaries will be estopped from claiming otherwise because of my representations to the Released Parties.

This release does not apply to any claims arising from the Released Parties’ gross negligence, reckless conduct, or intentional harm.

I acknowledge this waiver is supported by my trip deposit and acceptance into the dive trip (consideration).

If a minor is participating, I am the parent or legal guardian and consent to this Agreement on their behalf.

I understand that important updates and details regarding the trip will be provided via email and I agree to read and respond promptly to ensure smooth planning and travel.

I certify that I am physically and medically fit for the activities I choose, will disclose information needed for safe participation, will remain within my training and experience, and will follow the directions of dive professionals, vessel crew, and trip staff.

I understand this trip uses the following Trip Builder requirements: diver medical questionnaire — Optional; medical professional sign-off — Optional. A medical document is current for one year from its signature date and must remain current through the trip return date.

I am responsible for the identification and entry documents required for this trip. Trip guidance: For U.S. citizens traveling domestically (this trip is domestic), a passport is not required. Airline travel requires REAL ID‑compliant photo ID or another TSA‑acceptable ID. (Assumes travelers depart from the United States.) Not applicable for U.S. citizens on domestic travel. International visitors should check travel.state.gov for their visa requirements.

If I cannot consent during an emergency, I authorize reasonable first aid, evacuation, medical assessment, and treatment. I understand I remain responsible for related expenses not paid by insurance.

I agree to follow safety rules, local law, resort and vessel policies, environmental requirements, and staff instructions. Unsafe, unlawful, abusive, or materially disruptive conduct may result in removal from an activity or the trip without reimbursement for unused services.

Deposit: 100 — applied toward the trip balance. Deposit due: On booking. Final payment due: 2027-01-01 (Trip Builder). Full payment is due at booking for reservations made on or after this date.

I understand that Triangle Dive Shop/Water World reserves the right to adjust trip details or pricing if necessary. In the rare event of a price increase, I will have the option to cancel my reservation for a full refund of monies paid.

I understand that Triangle Dive Shop/Water World acts as an agent for hotels, resorts, dive operators, airlines, transportation services, and other independent providers, and is not responsible for their acts, omissions, or failures. I acknowledge that any disputes, claims, or issues arising from services provided by third parties must be resolved directly with those providers.

I agree that, when necessary due to weather conditions, mechanical issues, local laws, or safety concerns, Triangle Dive Shop may substitute comparable accommodations, vessels, staff, dive sites, or trip components. No refunds will be provided for substitutions or services unused by personal choice.

I understand that unused or missed transportation, lodging, diving, training, meals, or activities caused by my late arrival, early departure, illness, conduct, documentation problem, or personal choice may not be refundable, subject to the trip-specific cancellation policy and applicable law.

Trip retainer: non-refundable. Cancellation within 120 days of departure: 50% refund. Cancellation within 60 days of departure: no refund. For cancellation earlier than 120 days, contact Triangle Dive Shop for the applicable supplier terms. A specific cancellation policy in Trip Builder takes precedence.

To the extent permitted by law, this agreement is governed by North Carolina law and any permitted dispute will be brought in a court with jurisdiction where Triangle Dive Shop operates.

I certify that all information I have provided is accurate and truthful. I have read and fully understand the terms, conditions, policies, waivers, and responsibilities outlined in this agreement and I agree to abide by them as a participant in the above-referenced trip.

I hereby grant permission to Triangle Dive Shop/Water World, its representatives, and affiliated partners to photograph, video record, or otherwise capture my image, likeness, voice, or written testimonials during the course of this trip. I authorize Triangle Dive Shop to use, reproduce, publish, and distribute these photographs and recordings in any format—including print, digital, online, or social media—for marketing, promotional, educational, or commercial purposes without compensation to me. I understand that my name will not be used in association with my image or likeness without my separate, written consent unless otherwise required by law. I acknowledge that I have no right of approval or claim to royalties or other compensation arising from the use of these images or recordings.

I authorize Triangle Dive Shop/Water World to process payments using the payment information I have provided for this trip on the agreed upon schedule. Payment processing remains in DS360; do not enter payment credentials in this form.

I will confirm my travel and dive insurance arrangements before departure. If I decline coverage, I accept the financial risks under the cancellation policy shown here. Insurance and medical requirements set by the trip team or provider still apply. Signing this agreement does not certify travel readiness or medical clearance.
